A Support Coordinator:

    • Works with you to determine what NDIS funding you have available for services. 
    • Assists you to identify what you would like to get out of your services and how, when and where you would like these delivered. 
    • Whenever possible, supply you with a range of potential service providers, to enable you to make an informed choice. 
    • Assist you to identify service providers in your area that are most likely to meet your needs. 

Coordination of Supports 

Assist you to set up your services, continue to monitor and manage service progress against your stated goals, and step in where required to assist you to resolve any service issues. 

Specialist Support Coordination 

Qualified practitioners can provide specialist advice and support to those individuals with high level or complex needs (this will be recognised and funded in the participant’s NDIS Plan). We will assist you with any challenges or barriers you may experience and ensure that you are realising the full benefits of your Plan. 

Under the NDIS, Support Coordination is known as Capacity Building. It will be listed in your NDIS plan under this area. Eligibility for NDIS funding for support coordination is determined at your NDIS Plan Review meeting.

Yes. Your Assured Home Care Support Coordinator can assist you in a number of ways including:

  • Preparation for the NDIS Plan Review meeting
  • Attending the NDIS Plan Review meeting with you on the day and
  • After your NDIS Plan Review meeting to assist with any follow up required. 

Yes. All you need to do is contact your existing Support Coordinator to tell them you want to end the arrangement with them and then contact us and we will arrange the rest. Please note all providers will have different notice periods and this can usually be found in your service agreement.
